Resumen

El sistema sanitario actual es el fruto de su evolución histórica. Los procesos políticos que acontecían en el momento en el que se fundan sus instituciones afectan decisivamente en su futuro desarrollo. Esta influencia queda patente en sus sistemas organizativos y se evidencia hasta en el diseño arquitectónico de sus edificios.
El sistema de las tres "C" propone un sistema sanitario centrado en el demandante de cuidados y que garantice un paso del mismo, por los distintos equipos de atención sanitaria, sin que perciba escalones o discontinuidad en los cuidados.
Se propone para ello, la no jerarquización de la asistencia sanitaria y enfatizar en la correcta transmisión de la información entre los distintos equipos, mediante el uso de las tecnologías que ya están accesibles.

Abstract (Communication and Continuity of Care. The three "Cs" system)

The present health system is the result of its historical evolution. The health system's actual foundations were laid on the political processes and circumstances existent in each historical moment, significantly affecting its future development. For instance, these factors did influence the organisational services and even the architectural design of its buildings. The three "Cs" system proposes a health system based on the subjects of care, which guarantees them safe passage throughout all the different levels of health care. In order to achieve this, a non-hierarchied health system must be implemented, where communication between the different health care teams and levels is encouraged and facilitated, and the last available technologies are made accessible.
