Depresión en pacientes con nefropatía diabética y su importancia en el diagnóstico enfermero

Resumen

Este estudio tiene el propósito de examinar la frecuencia de depresión en una muestra de pacientes con insuficiencia renal crónica (IRC) por nefropatía diabética (ND). Se realizó una investigación Expost-Facto, de tipo transversal, donde se tipifican sus particularidades. Los sujetos que participaron en el estudio fueron 100 pacientes que acuden al Programa de Diálisis Peritoneal Continua Intermitente de un Hospital General del Sector Salud de la Capital del Estado de San Luis Potosí, diagnosticados por sus médicos tratantes con secuela de nefropatía diabética. La presencia de depresión se midió con el instrumento GHQ-28 (General Health Questionnaire) en su versión validada, confiable y estandarizada al español. La variable dependiente a considerar fue la evaluación del índice de depresión estimado a través de la escala GHQ-28; la variable independiente fueron aspectos sociodemográficos y la condición crónica degenerativa de las complicaciones por nefropatía diabética. Los resultados revelaron que un 86% de la muestra estudiada de los pacientes con nefropatía diabética cursa con algún grado de depresión; teniendo más riesgo de padecerla, aquellos pacientes con más de diez años de habérseles diagnosticado diabetes mellitus tipo 2. Se concluye que la depresión en pacientes por nefropatía diabética es alta y asociada a la evolución degenenerativa del padecimiento. Se resalta la importancia de la intervención de enfermería en este tipo de pacientes.

Abstract

In order to test the frequency of depression in a sample group of patients with Chronic Renal Insufficiency (CRI) due to Diabetic Nephropathy (DN), an Expost-Facto study, of transversal type, was done. In this study it is typified every particularity about this problem. The subjects that participated in this study were 100 patients that attend to the continuous Intermittent Peritoneal Dialysis Program, in a public hospital from the health sector in San Luis Potosi. The diagnose of Diabetic Nephropathy was done by their own doctors. The presence of depression was measured with the GHQ-28 instrument (General Health Questionnaire), and the validated, trustful, and Spanish standardized version was used in this study. The dependant variable to consider was the evaluation of the index of depression estimated through the GHQ-28 scale; the independent variable was the socio-demographic aspects and chronic degenerative condition of the complications due to diabetic nephropathy. The results showed that 86% of the studied sample in patients with diabetic nephropathy deals with a level of depression; having more risk of suffering this problem those patients with more than ten years since they were diagnosed with Diabetes Mellitus Type 2. The conclusion is that depression in patients by Diabetic Nephropathy is high and it is associated to the degenerative evolution of this health problem. The importance of nursing intervention is stood out in this type of patients.
