RESUMEN

Introducción: El tabaco, de todas las drogas, es la que mayor daño sanitario genera. Sus efectos nocivos han sido advertidos desde hace más de 35 años en informe del Royal College inglés (1962). Actualmente, numerosos estudios han evidenciado los efectos nocivos y establecen asociación entre consumo y morbi-mortalidad. A corto plazo, entre los beneficios del abandono destacan mejoría de salud, aumento del gusto olfato y, en semanas, niveles más bajos de estrés. El beneficio depende del consumo, del estado de salud actual y del tiempo transcurrido desde el abandono.
Objetivos: Evaluar ganancia de peso en dos grupos de deshabituación tabáquica a 3 y 6 meses, diferenciar según tratamiento, monitorizar abstinencia y describir hábitos dietéticos tras taller nutrición.
Metodología: Estudio descriptivo observacional. Criterios de inclusión: mayor de 18 años y fumador activo. Exclusión: enfermedad psiquiátrica, embarazo y otras drogas. Abordaje farmacológico y cognitivo-conductual: 10 sesiones en 3 meses y seguimiento a 4 y 6 meses. Control de peso, tensión arterial y síndrome de abstinencia. Taller sobre nutrición saludable.
Resultados: 28 personas, edad media: 44,4±6,6 años. A los 3 meses: 64,3% ganaron peso (1,8±1,4kg) y 35,7% perdieron (1,2±1,1kg). A los 6 meses: 71,4% ganaron peso (2,2±1,8kg) y 28,6% perdieron (2,2±2,1kg). Por sexo: 77,8% de mujeres engordaba a los 3 y 6 meses, y 57,9% hombres a los 3 meses y 68,4% a los 6 meses. Tratamiento: 46,4% vareniclina (92,3% abstinencia), 21,4% bupropion (66,7% abstinencia) y 32,1% terapia sustitutiva con nicotina (TSN) (77,8% abstinencia). Tratamiento-aumento de peso: 53,9% con vareniclina (80% mujeres-37,5% hombres). 66,7% con bupropion (50% mujeres-75% hombres). 77,8% con TSN a 3 meses y 100% a 6 meses (100% mujeres-hombres: 71,4% a 3 meses y 100% a 6 meses).
Discusión: La educación sanitaria nutricional en programas de deshabituación tabáquica consigue evitar aumento de peso. Enfermería contribuye en control de obesidad y mitiga los miedos que el abandono del tabaco tiene arraigado en las creencias populares.
Conclusiones: Debemos preparar un programa de educación sanitaria, para los pacientes en deshabituación tabáquica con el fin de mejorar su calidad de vida.

ABSTRACT
Relation between the To leave tobacco and the profit of weight

Introduction: The tobacco, of all the drugs, is the one that major sanitary hurt generates. His harmful effects have been warned for more than 35 years in report of the Royal College Englishman (1962). Nowadays, numerous studies have demonstrated the harmful effects and establish association between consumption and morbi-mortality. In the short term, between the benefits of the abandon emphasize improvement of health, increase of the taste smell and, in weeks, lower levels of stress. The benefit depends on the consumption, on the current bill of health and on the time passed from the abandon.
Objective: To evaluate profit of weight in two groups of deshabituación tabáquica to 3 and 6 months, to differ according to treatment, to monitor abstinence and dietetic habits describe after workshop nutrition.
Methods: Descriptive study observacional. Criteria of incorporation: major of 18 years and active smoker. Exclusion: psychiatric disease, pregnancy and other drugs. Pharmacological and cognitive - behavioral boarding: 10 meetings in 3 months and follow-up to 4 and 6 months. Control of weight, arterial tension and syndrome of abstinence. Workshop on healthy nutrition.
Results: 28 persons, middle ages: 44,4±6,6 years. To 3 months: 64,3% gained weight (1,8±1,4kg and 35,7% lost (1,2±1,1kg). To 6 months: 71,4% gained weight (2,2±1,8kg and 28,6% lost (2,2±2,1kg). For sex: 77,8% of women was putting on weight to 3 and 6 months, and 57,9% men to 3 months and 68,4% to 6 months. Treatment: 46,4% vareniclina (92,3% abstinence), 21,4% bupropion (66,7% abstinence) and 32,1% substitute therapy with nicotine (TSN) (77,8% abstinence). Treatment - increase of weight: 53,9% with vareniclina (80% women - 37,5% men). 66,7% with bupropion (50% wom 75% men). 77,8% with TSN to 3 months and 100% to 6 months (100% women - men: 71,4% to 3 months and 100% to 6 months).
Discussion: The education for the nutritional health in programs of deshabituación tabáquica manages to avoid increase of weight. Infirmary contributes in control of obesity and mitigates the fears that the abandon of the tobacco has established in the popular beliefs.
Conclusions: We must prepare a program of sanitary education, for the patients in deshabituación tabáquica in order to improve his quality of life.
