RESUMEN

Se presenta el caso de una mujer de 33 años diagnosticada de enfermedad inflamatoria intestinal con afectación colónica y probable colitis ulcerosa, corticodependiente, con fístula perianal intervenida y pérdida de eficacia del tratamiento farmacológico, síndrome ansioso depresivo. Paciente reticente al tratamiento quirúrgico, por lo que se opta por alternativas no quirúrgicas, no consiguiéndose resultados satisfactorios. Por lo que se decide la realización de panproctocolectomía total con reservorio ileal en J, con ileostomía lateral sobre varilla en fosa iliaca derecha.
Se elaboró un plan de cuidados siguiendo los patrones funcionales de M. Gordon y usando las interrelaciones NANDA NIC-NOC. En éste se hizo especial hincapié en el patrón de Autopercepción-autoconcepto, ya que se consideró básico mejorar el trastorno de la imagen corporal, la ansiedad y el temor. Queremos destacar que, a pesar de haber estado la paciente implicada en la toma de decisiones sobre su proceso en todo momento, en el postoperatorio presenta un rechazo sistemático hacia su estoma. Al mejorar este patrón se consiguió una mejora llamativa en la evolución de la paciente y en su proceso global de recuperación.

ABSTRACT
Care Plan for a young patient with contempt for her stoma

We present the case of a 33-years-old woman diagnosed with inflammatory bowel disease with colonic inflammation and probable ulcerative colitis, with corticosteroid dependency, perianal fistula submitted to surgery and loss of effectiveness of pharmacological treatment. She also presents a depressive anxiety disorder. As the patient was reticent to surgical treatment, nonsurgical alternatives were tried, not achieving satisfactory results. For this reason, the decision was taken to carry out a total panproctocolectomy with ileal J pouch associated to an ileostomy with rod in the right iliac fossa.
We developed a care plan following functional patterns M. Gordon and using the inter NANDA NIC-NOC. In this plan we did special emphasis on the pattern of self-awareness and self-concept, as they were considered basic to improve the patient´s body image disorder as well as her anxiety and fear. We want to emphasize that, despite the patient being involved in making decisions about her process at all times, in the postoperative period she presents a systematic contempt with respect to her stoma. When this pattern improved, remarkable improvements in the evolution of the patient and her overall process of recovery were achieved.
