Eficacia del triaje en centros de atención primaria

Resumen

Introducción: La atención primaria representa un papel fundamental en la sanidad, es el primer contacto del individuo con el sistema sanitario y el primer contacto del profesional con el paciente y su problema. Debido al incremento de la demanda en los centros de atención primaria es necesario pensar en estrategias para intentar llevar a cabo una mejor organización asistencial, sin saturar demasiado al trabajador. Objetivo: Como objetivo general se plantea comprobar la eficacia de implantación del triaje en el sistema sanitario de atención primaria. Para ello, se pretende verificar que, en un centro de salud sin triaje, un paciente con una urgencia grave, tardaría más en ser atendido que en un centro de salud con triaje. Comprobar que, sin la estructura del triaje, el usuario ha de deambular más entre consultas y mostrador para registro de pruebas y datos. (desde que es registrado por el administrativo hasta que es dado de alta). Método: Este trabajo se centra en la organización de las urgencias o pacientes que acuden sin cita previa al centro de salud a través del triaje enfermero. Se optó por utilizar como referencia el triaje �Manchester- Deimos�. Se registran las demandas de los pacientes y se seleccionan las que son más urgentes. Concretamente las no demorables más de diez minutos, con una clasificación de color naranja en el triaje �Manchester�. Posteriormente, se analizan y comparan los tiempos de demora y circuitos internos a los que se somete el paciente durante su proceso asistencial y la capacidad de actuación de enfermería para la resolución de las urgencias más leves. Resultados: De las doce demandas registradas, cuatro fueron urgencias relevantes. Al finalizar el estudio, se puede comprobar cómo en los centros de atención primaria sin triaje existe una demora mayor del paciente con urgencia grave. Incluyendo las diferentes derivaciones a las que es sometido. Conclusiones: La principal ventaja es que la instauración del triaje proporciona una información clara al usuario y al profesional sobre su estado y el tiempo aproximado de espera, ayudando, en la selección de emergencias ocultas. El establecimiento de un sistema de priorización en los centros de primaria mejoraría la organización de la demanda de atención urgente. ¿Qué contribución tiene este trabajo a la Enfermería? Este proyecto intenta buscar un método que ayude y mejore la eficacia de la organización de las numerosas demandas (sin cita previa) del usuario en atención primaria. Por otro lado, potencia la figura de enfermería, como se verá más adelante.

Abstract
Effective triage system in primary care

Introduction: most of the times, the first contact of patients with both, the health system and health professionals occurs in the context of the primary health network, therefore, the role of these facilities is essential in the health context. The increase in the demand of primary care services require the development of new strategies for a better management of healthcare, but care must be taken not to overload health staff. Objectives: the main objective of this study would be to check the effectiveness of the implementation of a triage system in primary health centres. We would try to confirm that a patient presenting with an emergency would take longer to be attended in health centres with no triage system, where he may spend more time giving all the medical and administrative information required from different professionals (since he is admitted at the entrance with triage system. Methods: we will focus in the nursing management using a triage system of those patients coming to the health centre due to an emergency or simply without an appointment. We decided to use the �Manchester-Deimos� triage. The demands of patients are registered and the ones that are more urgent (classified as orange according with the �Manchester� triage) are selected as they can not be delayed more than ten minutes. Later, we assess and compare different parameters such as: waiting time, internal procedures that patients have to follow and the nursing abilities to solve less urgent emergencies. Results: we registered twelve demands of assistance, being four of them urgent. With this study we show that in those primary health centres which did not use a triage system patients with a serious problem had to wait longer; including the different patient referrals. Conclusions: the main advantge of the implementation of a triage system is that provides both the patient and the professional clear information regarding their health condition and approximate waiting time, and at the same time help to detect hidden emergencies. A triage system improves not only the management of urgent assistance but also the organization of the large number of patients that come to primary health centres without previous appointment. Being the nurse the professional in charge of this triage system their role would strengthen.
