¿Qué papel tiene enfermería en la valoración y manejo de la analgesia, sedación y delirio del paciente crítico con ventilación mecánica?

Resumen

Objetivo principal: Conocer percepción, opinión y conocimientos de enfermería sobre valoración y manejo de analgosedación y delirio. Concienciar sobre la importancia de su implicación. Consolidar y actualizar conocimientos. Metodología: Estudio observacional analítico transversal realizado en cuidados críticos, abril-noviembre 2011. Se realizaron 9 sesiones formativas entregando antes-después test de conocimientos y cuestionario de actitud. Significación p<0,05. Resultados principales: Se entregó cuestionario a las 143 enfermeras/os, edad 33 (28-40) años. Conocían herramientas validadas para valorar sedación, analgesia y delirio el 59%, 57% y 2%, no las utilizaban el 77%, 79% y 100% respectivamente. La puntuación del test de conocimientos se incrementó (p<0,001) 3,5 (2,1-4,9): 8,5 (7,8-8,5). En 11 preguntas de percepción y 3 de opinión las enfermeras modificaron sus respuestas hacia el extremo opuesto (p<0,01) y hacia el mismo extremo en 6 preguntas de opinión (p<0,001).  Conclusión principal: La difusión de herramientas para valorar analgosedación es aceptable pero su aplicación insuficiente. Existe gran desconocimiento sobre como valorar el delirio. La formación proporcionó una visión más realista del manejo de la analgosedación y delirio, incrementó conocimientos y concienció sobre la importancia de la implicación de enfermería.

Abstract (What role does nursing have in the assessment and management of analgesia, sedation and delirium of critical ill patient on mechanical ventilation?)

Objective: Learn perception, opinion and knowledge of nursing about assessment and management of analgesia, sedation and delirium. Raise awareness of the importance of their involvement. Consolidate and update knowledge. Methods: Observational analytic cross-sectional study performed in critical care, abril-noviembre 2011. Nine training sessions were conducted, delivering before-after test of knowledge and attitude questionnaire. Significance p <0,05. Results: Questionnaire was given to 143 nurses, 33(28-40) years old. They knew validated tools to assess sedation, analgesia and delirium 59%, 57% and 2%, not the used 77%, 79% and 100% respectively. The knowledge test scores increased (p<0.001) 3,5 (2,1-4,9): 8,5 (7,8-8,5). Nurses change significantly their answers in 11 questions of perception and 3 of opinion onto the opposite end (p<0.01) and toward the same end in 6 questions of opinion(p<0.001). Conclusions: The dissemination of tools to assess analgesia-sedation is acceptable but their implementation is inadequate. There is a great lack of knowledge on how to assess the delirium. The training provided a view of management of the analgesia-sedation and delirium, increased knowledge and raised about the importance of the involvement of nursing.
