RESUMEN   Las condiciones de vida han variado tanto en los últimos años que están generando problemas de atención a poblaciones que tienen más necesidad de cuidados, donde un gran número son las personas mayores, están haciendo que la atención a nuestros mayores sea causa de enfrentamientos familiares, deterioro la integración social, aislamiento social, etc. Nuestra protagonista ingresó en el hospital por un proceso agudo, y cuando se resolvió su problema de salud y se propuso el alta hospitalaria, ella no quiso ir a su domicilio y empleó todos los medios a su alcance para alargar la estancia en el hospital, evitando así temporalmente la soledad en su domicilio.   Con las actuaciones enfermeras adecuadas, hemos conseguido preparar a un familiar, y a la propia usuaria para garantizar que se le presten los mejores cuidados domiciliarios, con una menor demanda de asistencia sanitaria y aumentando su independencia institucional. Midiendo los resultados obtenidos tanto en el individuo, como en la familia y comunidad. Para ello hemos contados con un lenguaje normalizado en el que están incluidos los diagnósticos enfermeros (según taxonomía NANDA), la Clasificación de intervenciones de enfermería (CIE) y de resultados esperados (CRE). Abstract Deterioration of the social integration and solitude in the old age:  Clinical case   The conditions of life have as much varied in the last years that are generating problems of attention to populations that have more necessity of cares, where a great number are the greater people, they are causing than the attention to the our greater ones is cause of familiar confrontations, deterioration social integration, social isolation, etc. Our protagonist admitted the hospital by an acute process, and when its problem of health was solved and the hospitable discharge seted out, she did not want to go to her address and used all means to her reach to temporarily extend to the stay in the hospital, thus avoiding the solitude in her address.   With the suitable performances nurses, we have been able to prepare a relative, and to the own user to guarantee that the best domiciliary cares are lent to him, with a smaller demand of medical assistance and increasing its independence institutional. Measuring the results obtained as much in the individual, as in the family and community. For it we have counted with a standardized language in which they are including the diagnoses nurses (according to taxonomy NANDA), the Classification of interventions of infirmary (CIE) and awaited results (CRE).
